Requirements for Affiliated Centers:
Each FertilityCare Practitioner who provides CREIGHTON MODEL FertilityCare System (CrMS) services must do so under the auspices of a FertilityCare Center affiliated with FCCA.
Each FCCA-affiliated center must identify one FertilityCare Practitioner as the “Responsible Practitioner” who is charged with ensuring the center complies with FCCA standards. The Responsible Practitioner has three years from the completion of their education program to obtain (or be in the process of seeking) certification from the American Academy of FertilityCare Professionals.
To become certified as a Creighton Model FertilityCare System Practitioner, Instructor, Educator, Supervisor, Medical Consultant, Nurse Practitioner, Physician Assistant, Nurse Midwife, or Pharmacist, contact the AAFCP.
